The door itself is 12 x 12+3/16 with rounded corners and it needs the frame to fit to the water heater and the wall. The exterior dimension of the frame that the door fits into is 15"x. The door has the opening for the latch in the correct location, about one inch left of the center.

The door has holes for two pins on the bottom and the top latch to hold it closed. There’s no plastic it’s aluminum or some type of metal but yes it comes with a sleeve that slides in and screws into the sides of the water heater. This is to prevent freeze damage.
